A Study of Cinical Significance on the Detection of Human Papilloma Virus in the Head and Neck Cancers


Abstract
To analyze the relationship of the presence of HPV genome in tissues from patients with head and neck cancers and prognosis, and availavbility for using as parameter in prognosis, PCR was performed with the 63 biopsied specimens (larynx 36,
hypopharynx
6, oral cavity & oropharynx 12, tongue 7, sinonasal cavity 2) by using the L1 concensus primer. The type of HPV was determined with the same method with HPV -16, -18, specific E6 primer that can amplify the specific site of HPV genome. We
obtained
the
following results from the analysis of the data such as detection of HPV DNA, type of HPV, and clinicopathological findings.
1. HPV DNA was detected in 12 cases among the 63 head and neck cancer patients (19.0%) and according to the type of HPV DNA -16 was detected in 5 cases (7.3%) , HPV-18 in 2 cases (2.9%), non -HPV -16, -18 in 6 cases (8.8%).
2. In laryngeal cancer, HPV DNA was detected in 8 cases (22.2%) out of the 36 patients, and HPV -16, HPV -18, and non-HPV-16, -18 were detected in 5 cases (12.3%), 1 cases (2.5%) 3 cases (7.4%), respectedly.
3. HPV DNA was detected in 2 cases (16.6%) of the 12 oral cavity and oropharynx, in 1 cases (14.2%) of the 7 tongue cancer, in 1 case (50%) of the 2 sinonasal cavity, and types of HPV were non-HPV-`6, 18 except 1 case of HPV-18 dectected in base
of
tongue cancer, but HPV was not detected in 6 hypopharyngeal cancers.
4. There was no correleation between the detection rate of HPV DNA and clinicopathologic parameters.
From these results the dectection rate of HPV-16 is higher than the other site, but the absence of detectable HPV DNA in the majority of carcinoma suggests that HPV is not the major cause of head and neck cancers, but in some cases of cancers
HPV
might
play an etiologic role in the carcinogenesis of head and neck cancer.
